Please note, however, upon review I have found a number of what I believe are typographical errors in the text of the request as it appears in the first paragraph of your letter.

I am concerned that these errors may result in no records being found and, therefore, I kindly request you update/correct the text of the request, then re-execute it.

The errors are as follows:

  1. My name is incorrectly stated as “FOX, Patick”. The “R” is missing from my first name, PATRICK.
  2. My prior name is incorrectly stated as “FOS, Richard Riess”. It should have been “RIESS, Richard”.

Also, I'm not sure if it matters, but I believe the substring:

should have been:
(aka RIESS, Richard)

For your convenience, and for clarity, I believe the entire request text should be as:
