Dear Mr. Poll:

Please find, enclosed, my Application to a Judge form regarding my application for a disclosure order and witness subpoenas.

Since I do not have the ability to make or obtain photocopies at NFPC I am forwarding this to you and requesting that you attach to the form the following correspondence between you and I, because I believe that correspondence adequately states my requests and your responses to/denials of those requests.

My 2022-07-25 letter to you;
Your 2022-08-16 letter to me;
My 2022-08-18 letter to you;
My 2022-09-02 letter to you;
Your 2022-09-13 letter to me.

Then, may you please file the application form, with those letters attached, with the court? I would do it myself but, you know, I'm in jail and all that.

Then, may you or the court kindly return a copy for my records?
